What to Expect
Introduction
I am often initially contacted by the adult children of clients in need, either directly, or through referrals from senior living professionals, physicians, geriatric care managers, or other healthcare providers.
Assessment
I will meet in person with the client and/ or their loved ones to gain a thorough understanding of their needs. This comprehensive initial assessment may include, but is not limited to, a physical exam, a thorough review of the client’s medication list and medical history, a safety evaluation of their living environment, and documentation of other healthcare providers involved in their care team so that coordination and collaboration can begin when applicable.
Care Plan
Post assessment, I will be able to develop a detailed, personalized care plan, ensuring that North Shore Nurse provides the highest level of support tailored to the client’s specific requirements.
Client
Once the care plan is established and agreed upon, North Shore Nurse takes proactive steps to implement efficiently and effectively. This not only includes executing the outlined care plan objectives, but also coordinating with other healthcare providers involved in the client’s care, keeping family or other responsible parties informed as requested, and always acting in the client’s best interest to achieve the best possible outcomes.
